A facility where courteous trained and experienced staff assists residents with as much or as little assistance as they require or request. This assistance includes personal grooming and hygiene, observing and assisting with medications in order to assure that the medications are taken in accordance with doctors’ orders, making sure that the residents nutritional needs are met all in a safe, secure environment.

The Graceful Swan PCH provides high quality, safe, secure, reliable, and cost-effective  home with supportive services to our residents. All staff have under gone and passed comprehensive background check.

We assist our residents and their families with the transition from hospital, or rehab facilities to our home through comprehensive planning and coordination with hospital/rehabilitation facility staff and physicians.

Our staff are here for the comfort and well being of our residents. All rooms are furnished with a bed, bedside table, dresser and chair. Linens and towels are provided. We suggest you bring your own special personal items to make your room feel like its yours. Pictures, chairs and other items from home are ways of making a room unique.

With a limited number of residents in The Graceful Swan PCH, we can then give you the attention you deserve and provide you with a home that is quiet and personal.
