Courtyard Estates of Sullivan is an alternative level of quality housing appropriate for many seniors who choose not to live alone. At Courtyard Estates, we will create a service plan for each tenant upon admission. The service plan is updated regularly to assure that the tenant receives the appropriate care according to his or her needs and preferences.
It is our goal for all area seniors to take advantage of the full range of services available. If continuing care needs evolve to include a higher level of nursing services and relocation is imminent, our trained professionals will be there to assist with the transition.
Courtyard Estates of Sullivan is focused on keeping the senior population as active as possible. Our commitment to each tenant begins the day they become part of our family. After conducting a comprehensive assessment, to better understand you or your loved ones level of activity, a specialized care plan will be established. This allows the Courtyard Estates staff to assist each tenant with the level of activity they are comfortable with.
Active Plus Care is the entry level for tenants within the independent wings of our Senior Community. The Active Plus Care level is designed to maximize individual freedom and choice, providing just a little help in the tenants daily life. Services include:
- Daily dietician planned meals
- The choice of weekly housekeeping and linen service
- Daily trash pick-up and bathroom cleaning
- Planned activities and educational events
- Social events
- Community and family participation in activities
- Emergency call system (monitored 24 hours per day)
- Tenant safety checks (24 hour staff on premises)
Assist Plus Care is designed to accommodate an individual tenant’s changing needs and preferences. As needs change, the health care team, in conjunction with the tenant and his/her family, will review and appropriately adjust the service level as necessary. Services include all of Active Care plus the following:
- Stand-by assistance with bathing and showering as needed
- Individual assistance with dressing and personal grooming as needed
- Daily medication reminders
- Assistance to and from dining room and activities
- In-room dining during minor illness (at nurses discretion)
- Free transportation to and from medical appointments
