Windy Acres Long Term Care

Windy Acres is located on 5 acres in Forest Lake, MN on highway 8. The grounds are mostly enclosed with a chain link fence and a completely fenced day yard preventing access to the highway. A security camera is located at the entrance of the building, in addition to 3 others inside the facility. All doors are equipped with ramps for handicap access. Alarms are wired on all entrance and exit doors.

The building is comprised of 14 private rooms and 2 double rooms. All rooms are easily accessible to the nurse’s station and bathroom facilities. There is a community dining room, living room and an enclosed deck with a door alarm located off the living room. Smoking is allowed outside on the deck or in the back yard of Windy Acres.

We believe that providing frequent opportunities to socialize amongst peers and interaction with staff in meaningful and stimulating ways specifically grounded in reality orientation provide opportunities to focus on the remaining strengths of the resident while helping to preserve dignity and self-respect.

Windy Acres accepts residents with dementia and Alzheimer’s disease as diagnosed by their physicians. Acceptance criteria for placement includes those residents with short term memory loss, inability to retain new information, decreased ability in decision making and planning, the socially withdrawn, those becoming neglectful of health and hygiene issues and those residents requiring frequent reality orientation.

During the pre-admission conference, data will be gathered from the family, M.D., and any previous placements to generate a service plan unique to each individual. This plan will indicate what types of services are deemed to be necessary, the frequency of the service, who will perform the service, supervision of those persons performing the service and any additional fees required beyond the per day charge.

As a resident’s condition changes while residing at Windy Acres, the staff will document those changes. When the changes appear to be permanent, a modification of the service plan will be written and given to the resident's responsible person and the staff adapt their care according to the resident's changing needs.

Windy Acres provides daily activities Monday thru Friday by a program director, designed to stimulate memory and reality orientation. Other daily activities include television news programs, one-on-one visits with staff and volunteers, a daily calendar, afternoon social hour, reading the daily newspaper to highlight the daily headings, and inviting interactive participation of the residents. Several times during the week residents are invited to participate in board games in order to stimulate memory and provide socializing opportunities. Each month we have different individuals or groups come and play the organ and other instruments and involve residents in old fashioned sing along.
