CCHS is a provider of home and community-based services for adults with developmental delay, mental illness, traumatic brain injury, or other disabilities. Services can be provided within our facilities to ensure that they receive 24-hour supervision and monitoring. Our patients are cared for in a family-like atmosphere by trained and loving staff.

Major Roles of Our Care Professionals

Chase Courageous Health Services provides home health aide services to help nurture our patient’s health and independence while staying comfortable and safe at home. Rest assured that our board-certified home health aides are experienced in caring for elders and patients with disabilities.

Chase Courageous Health Services offer medical social work for our patients who cannot do it on their own or if their family is, otherwise, unavailable. This service is designed to provide necessary support and resources to our patients and family members, which are deemed essential for their recovery and rehabilitation.
