Welcome to Chestnut Hill, a 111- bed skilled nursing facility and a 125-bed assisted living facility located in Passaic, New Jersey. Our staff and services offer excellence and quality of life throughout the continuum of care. Our services include short term sub-acute rehabilitation, skilled nursing care, hospice, respite, assisted living, dementia and long term care.

Selecting a facility for your loved one is a major decision. Although quality care and service is a must, being in a comfortable, homelike environment is equally important. Chestnut Hill’s goal is to ensure that residents maintain their independence by motivating them to reach their personal best; and by providing the assistance and services each resident requires to maintain their independence for as long as possible.

Chestnut Hill takes an interdisciplinary approach when it comes to managing the resident’s short term or long term healthcare. Each resident is assessed by an experienced healthcare team who then develop a plan of care individualized to meet that resident’s needs. This approach ensures each resident is participating in a plan of care that will enable a smooth transition back to their routine activities of daily living.

Our Center features comfortable rooms that offer residents privacy and a quiet place to rest after a busy day of rehabilitation, socialization, and activities. Our sub-acute accommodations include free cable and flat screen televisions. Phone service is also available for a minimal cost.

At Chestnut Hill Residence Assisted Living, we provide the assistance your loved one requires to remain independent as long as possible. By providing loved ones with dignity, privacy, social interaction, three meals daily, housekeeping, activities throughout the day and trips to the store and elsewhere, families and friends can spend quality time with them rather than the quantity of time that was spent on providing care when their loved one lived alone.

The philosophy of the dementia special care unit at The Gardens in Chestnut Hill is to meet the individual needs of people living with Alzheimer's or related diseases. We strive to provide a home like environment in which residents can participate in daily living activities at their highest level of functioning. Residents are encouraged to take part in meaningful and appropriate activities through which they can feel pleasure and experience success.

The dementia unit provides space that is secure, comforting and non-threatening. It offers a familiar frame of reference, reduces confusion and offers freedom to move about in an unrestricted space. Within the Alzheimer's unit, highly trained staff creates specific programs that promote each patient’s capabilities fully, and minimize those stresses that can aggravate the symptoms of dementia and Alzheimer's
