Monthly rent in our Memory Care community includes a broad range of personal care services that ensure individualized care, comfort, and attention. In addition to the services offered in our Transitions™ Care program, we can also provide assistance with mobility, bathing, dressing, and incontinence care.
Our Transitions™ program is flexible in that it is designed to meet the ever-evolving personal care needs of those displaying the initial signs of memory loss. As one's needs become more advanced, so does the level of care and attention we provide.
Our unique program allows individuals with like needs to live together, which helps residents to maintain their highest level of independence and interpersonal relationships.
Our community consists of single-story residences designed for those who have difficulties with the activities of daily living due to memory loss. Each of our individual cottages feature home-like interiors that include family-style dining rooms, living spaces, and inviting patios that are enclosed for experiencing the outdoors safely. In addition, our activity programming is tailored to meet the changing needs of each of our residents.
Our goal at Farmington Square is to meet each resident's needs while maintaining the highest level of choice, dignity, privacy, respect, and individuality. To assure that this is done effectively, a personal assessment is conducted prior to move-in. Residents, family members and the Farmington Square staff work together to develop an individually tailored service plan. A follow-up family care conference will be held regularly to assure that, as a resident's needs change, the Personalized Service Plan is adjusted accordingly.
Personalized Service Plans may include:
- Daily assistance with hygiene, dressing and bathing
- Assistance with mobility to and from meals and activities
- Incontinence care
- Additional housekeeping
- Medication management: monitoring, supervision and assistance
- Safety checks
- Mealtime reminders
- Assistance with eating
- Periodic nighttime care
- Select diabetic care
- Health services: monitoring, supervision or assistance
- Coordination with outside services to help meet individualized needs

When the time comes, the Respite Care program at Farmington Square Medford offers a safe, supportive environment where personal and medical needs can be met by a skillful staff that is dedicated to caring for the needs of older adults. Our Respite program is available for short-term stays, for up to 30 days. Month-to-month leasing is available for longer stay needs.
