Maybe you ll find them playing cards, getting creative with arts & crafts or gathering for a movie. Each of these activities is enjoyable, yes, but they re part of a carefully designed care program to help our residents live their best life.

Because Brookdale Creekside specializes in memory care, our community has been designed with our residents in mind. With monitored entries and exits and an enclosed courtyard and garden, they remain safe while having complete freedom of movement. While our common areas are designed for socializing, our private apartments offer privacy and their own quiet place. Filled with natural light, our apartments are a place for your loved one to be surrounded by the comfort of his or her own familiar furnishings, family photos and treasured keepsakes.

Our memory care program is focused on creating feelings of belonging and purpose for your loved one, while seeking to preserve their sense of self. More simply, we meet them where they are. With gentle guidance and visual reminders, we help them flourish in activities such as gardening, kitchen work and other projects that provide the opportunity to use meaningful life skills and talents.

Our supportive dining environment encourages familiarity and socialization during mealtime. Three flavorsome, nutritious home-style meals and snacks are served daily in our dining area. We keep mealtimes very routine for our memory care community to help alleviate any confusion or added stress. All residents sit together at the same time and same table each day and place settings enhance and provide ease for each person.

Deeply rooted in a person-centered approach, our Alzheimer s and dementia care is called the Clare Bridge program, where each resident is surrounded by others with the same cognitive abilities, to give them a feeling of fitting in.

The Clare Bridge Daily Path provides residents with a structured, yet fun, routine of activities discussions for mental workouts, dancing for physical exercise that help maintain their abilities and encourage the use of their current skills, even with advanced expressions of the disease. The Daily Path is carefully structured to give each resident purpose and a sense of security that they are right where they need to be.
