At Second horizon Living, we specialize in care and daily living assistance to an array of individuals, that is tailored to meet the individual needs of each resident. Our reliable staff provide 24-hour supervision, 7 days a week, under the guidance of licensed nursing staff who are always available on-call.

We understand that not one care plan fits all. We take the time to thoroughly train our staff to provide daily services that can include anything from meal preparation, hygiene, cleaning, and supervision. We will take the time to get to know you and develop an individualized care plan that fits your specific needs. Not only do we focus on the medical needs, but on the person as a whole. In addition we provide 24 hour on-call nursing support as needed.

Along with direction from trained mental health specialists, our staff provide tools and supports to help individuals with mental health needs thrive in their current settings, and possibly develop the ability to transition to settings where they can live more independently.

We work with the clients to create personalized activity calendars and care plans that are tailored to meet their socialization needs and also provide transportation for clients needing to go out into the community

Physical disabilities and age-related diagnoses may create challenges completing day to day activities. as such we provide assistance with:

• Dressing and grooming
• Showering
• Assistance with transfers and ambulation... among others
• Mental Health Support and Other Skilled Services
• Skilled nursing services
• Medication administration
• Delegated nursing services
• 24 hour supervision
• On-call nursing 24 hours/7 days a week
• Setting up medical and non-medical appointments and transportation
• Counselling supports as needed
• Behavioral case management... among others
