Colonial Poplin and Poplin Way offer assisted living, skilled nursing, rehabilitation, and long term care services – all under one roof. We offer the senior community three main services, through our community assisted living residence and nursing home and rehabilitation center. Poplin Way is comprised of 18 large studios and one bedroom apartments. Our privately owned community assisted living community offers around the clock nursing assistance in a charming, country environment. Each apartment and the entire residence is designed to make residents feel completely comfortable, and truly at home.

Residents live in their own apartment, with that added benefit of having convenient access to skilled nursing care in the same location, when needed.

At Colonial Poplin we strive to continuously improve outcomes with a focus on a return to normal function. When a patient begins therapy, we devise individualized short term and long term recovery goals partnering with the patient’s at-home physicians. Our treatment based approach factors in history, current symptoms and clinical evidence in order to achieve maximum results and prevent re-hospitalization following discharge home.
